Ongoing assessment and planning with consumers

The organisation undertakes initial and ongoing assessment and planning for care and services in partnership with the consumer. Assessment and planning has a focus on optimising health and well-being in accordance with the consumer’s needs, goals and preferences.

3e Care and services are reviewed regularly for effectiveness, and when circumstances change or when incidents impact on the needs, goals or preferences of the consumer.	Care and services are recommended to be reviewed by the individual or support network in line with the individuals progress in their own therapy. All therapy schedules can be adjusted to suit the individuals' requirements at any given stage in their reablement. The Portable Hand Unit can be fully adjusted in intensity and time of the therapy to suit an individual's changing needs

3a Each consumer gets safe and effective personal care, clinical care, or both personal care and clinical care, that: i) is best practice; and ii) is tailored to their needs; and iii) optimises their health and well-being	The Portable Hand Unit is created with Cycloid Vibration Technology (CVT) devices, registered with the Department of Health's Therapeutic Goods Administration (TGA) and listed on the Australian Register of Therapeutic Goods (ARTG) as a class lla (2a) Medical Device. The person will be receiving a product that is built in Australia under best practice principles, is individually tailored and optimises health and well-being.
3c The needs, goals and preferences of consumers nearing the end of life are recognised and addressed, their comfort maximised and their dignity preserved.	The Portable Hand Unit has been specifically manufactured and clinically proven when used regularly to provide pain relief and increase circulation and blood flow- this will provide comfort and preserve dignity when a person is nearing end of life. It can be administered by the individual or the care team in place for end of life

3a Each consumer gets safe and effective services and supports for daily living that meet the consumer’s needs, goals and preferences and optimise their independence, health, well-being and quality of life.	The Portable Hand Unit can be individually tailored to suit the needs of a person- based on their current status and capacity. All needs are considered to ensure a person's independence, health and wellness are enhanced, and their opportunity to experience an optimal outcome is well supported.
3b Services and supports for daily living promote each consumer’s emotional, spiritual and psychological well-being.	The Portable Hand Unit strengthens an individual's health and wellness. Regular usage will support a person to reduce musculoskeletal pain and stimulate stronger blood flow, this in turn supports nerve and muscle activity and will provide promotion of individual wellness and well being and enable more opportunities for a person to engage in activities and events
3c Services and supports for daily living assist each consumer to: (i) participate in their community within and outside the organisation’s service environment; and (ii) have social and personal relationships; and (iii) do the things of interest to them;	The Portable Hand Unit strengthens an individual's health and wellness. Regular usage may assist a person with musculoskeletal pain and stimulate stronger blood flow, this in turn supports nerve, muscle and joint activity and will provide promotion of individual wellness and well being and enable more opportunities for a person to engage in activities and events
3g Where equipment is provided, it is safe, suitable, clean and well maintained.	The Portable Hand Unit is manufactured by an Australian company, Niagara Therapy, located in Parkinson Queensland. The product is made in Australia with the highest quality materials and hand crafted once an order has been placed. Produced to the highest standards. The equipment is not available in a rental agreement as it is to be delivered as a newly made, safe and suitable medical device.
